On March 29, 2005, plaintiff was sent a return of complaint form/direction to complete form requiring him to provide the name and address of a proper defendant. Plaintiff was also required to either pay the $25 filing fee or to file a poverty statement with supporting documentation. Plaintiff has failed to comply with the court orders. Therefore, plaintiff's action is DISMISSED, without prejudice, pursuant to Civ.R. 41. The court shall absorb the costs of this case. The clerk shall serve upon all parties notice of this entry of dismissal and its date of entry upon the journal.
